Payment Options

Clarity Healthcare offers a variety of payment options for your convenience. We accept the following insurance plans by state. Refer to your health insurance plan to determine coverage limits, copays and deductibles. If you do not have medical or dental coverage, we can offer a discounted fee structure based on federal poverty guidelines for individuals and families who qualify.

Sliding Fee Discount Program:

We want our services to be available to as many people as possible. Payment concerns shouldn't stop you from receiving the care you deserve. Our Sliding Fee Discount Program may be able to help you afford your services. To see if you qualify for a sliding fee discount, please call our billing department at 573-603-1460.
Note: if you have a ACA (ObamaCare) Marketplace plan, we may not be in network with your individual plan. Please verify coverage with our providers before your visit.

Transportation and Translation Assistance:

Transportation assistance for services may be arranged on a case by case basis, including transportation or specialty care.

Translation services are available for those who are hearing impaired or have limited English proficiency.
